Our office participates with a variety
of insurance plans. If you are a member of one of these plans, our
business office will submit the claim for services.
Please bring your insurance card to every
visit. Be prepared to pay your copay or any outstanding balance not
covered by your insurance plan. For your convenience we accept
cash, check, Discover, Visa or Mastercard.
Certain insurance companies require that
specific laboratories or hospitals should be utilized in order to
maximize your benefits. Please check with your insurance
company to see if this applies to you.
If you have questions about your
insurance, we are happy to help you. Questions about financial
arrangements should be directed to the billing department (269)375-6079. Specific coverage issues, however, should be
directed to your insurance company's member services department.
Our practice firmly believes that a good
provider/patient relationship is based upon understanding and
communication.
