In order to provide personalized,
effective medical care we must know as much as possible about your
current and past medical history. You will be asked to complete
patient registration forms prior to your first visit. These forms
will either be mailed to you or you may be asked to arrive 15 minutes
before your visit to complete these forms. Bring any pertinent
medical records with you, or have them sent before your first visit.
Obstetrical patients that are transferring into the office should have
their records here before their first appointment.
